EXOSOMES 38: Bell’s (Facial Nerve) Palsy
This brief blog is a summary of an interesting case of Bell’s Palsy that improved after MSC exosome injection. I will discuss what Bell’s Palsy is and present the case.
The facial nerve controls movements and sensations to the face. In most cases, the cause of Bell’s Palsy is unknown and in most cases, it resolves. Some people are treated with steroids and antivirals although the response rate is low because not all injuries are due to inflammatory or viral causes. Common associations are pregnancy, diabetes, and a history of infections at the time of onset.
The upper face is controlled by both cerebral hemispheres whereas the lower face is only the same side. That is why a stroke often spares the upper face whereas Bell’s Palsy, which attacks the nerve, affects both upper and lower.
The results are an inability to control the eyelids smile symmetrically and often botox is used to paralyze opposing muscles to keep the eyes open. The patient often requires eyedrops to keep the eye moist. Rarely, taste may be affected, there is increased sensitivity to sound, and there can be drooling.
I had previously treated a woman with minimal improvement by injecting here facial nerve plexus. Nevertheless, ES, a 69-yo patient with a 12-year history of stable Bell’s Palsy after an emotional stress wanted to try. In addition to injecting 1B exosomes into the facial nerve plexus, we also injected painful trigger points on her trapezius, paraspinal muscles (for scoliosis), and performed a lumbar epidural injection at L4/L5 for back pain. She has an “S-shaped” double scoliosis (side curvature) that appears to be causing asymmetrical stresses.
Overnight, the patient reported improvement in back and shoulder pains. (N.B. We generally don’t charge more for multiple sites of injection despite the increased effort and risks.)
Amazingly, her facial nerve sensations and control improved after just 10 hours and has continued to improve. Here is a photo of before (4 mos ago) and ten days after the injection.
I think it is fairly clear that in her baseline state of the last twelve years, her right eyelid drooped and she struggled to have a symmetric smile. She believes that her condition improved overnight. Do you agree?

Of note, I had previously treated another similar patient who only experienced minimal improvement. I think the “heaviness” is interesting and I explain that in the “soft signs” blog 27
Another interesting point is that nerve damage from emotional stress was also the main factor in KH’s case which showed improvement in her tinnitus (see blog 36).
By way of disclosure, I just treated another patient with tinnitus after sound damage and his condition did not improve. That makes the response rate only one in three thus far. So 50% for Bell’s Palsy and 33% for Tinnitus? I don’t think it is that simple. It may have to do with what caused the dysfunction and how prepared the subject is to reverse the damage. Still, with additional unexpected benefits and a near zero chance of improvement with existing therapies, the use of exosomes might help selected people when administered within a monitored patient-physician relationship.
